Within the inky blackness of space, a single world sits, crowded by moons, both artificial and natural, a giant ring of iron sitting around the planet, bloated cargo ships carrying ore to and fro the ring and the surrounding moons, carrying rare precious metals to the forges of the planet below, where upon billions of workers would spend their lives right up until their agonizing deaths toiling making new additions to the imperial war machine. Thousands would die a second from the ravenous atmosphere of the planet, as it was so polluted from all the pollutants constantly spilling into the atmosphere. It never rained here. It once had, thousands of years ago after the worlds first death when it had first been terraformed, but those days were long over. Colossal spires and cathedrals dedicated to the Machine god towered even higher than the polluted atmosphere of the planet, being visible even from space due to their size. This world was no hive world or otherwise, it was a forge world. Mighty planets which sole purpose was to produce weapons, tanks and other things for the imperial war machine. Governed by Tech-priests and protected by legions of skitarii, for in the grim darkness of the far future

It was bleak and unworthy, nothing to note, sensor spines and artificial sensing arrays set up around the forge worlds orbit detecting reems of empty data, being scrawled by hundreds of auto quill servitors, blankly copying onto scrolls of parchment; only to be recycled and used again.
But something did happen, a cluster of sensor spines had failed to report back any data, perhaps a malfunction in the machines spirits; or a neglect of the delicate machines.
It had gone unnoticed for a few days, no notice taken...

As soon as this is finally noticed by a tech adept over viewing the data input and output, a request is sent for a unit of menials to go and see what is wrong with the data spines

They would find the sensor spines damaged, broken parts floating in the void.
Damage looks to be caused by heat, metal panels fused and scorched together.
